Job Description:

The PRN Patient Relations Coordinator serves as a primary liaison in the facilitation of medical referrals and appointments for domestic and international patients to our hospitals and physician practices.
In collaboration with the patient associate, conducts case reviews and triages new referrals, and participates in the development, implementation, and facilitation of patient care along a continuum to help patients achieve optimal health assuring care is the right sequence and at the appropriate level.
Responsibilities:

• 
  
• Provide assistance to patients, physicians and other overseas referral sources with appointment scheduling, admitting, discharge and follow up.
  
• Leads and/or facilitates multi-disciplinary patient care conferences.
  
• Collaborates daily with finance/billing to ensure and verify benefits and authorization.
  
• Serve as patient advocate, ensuring that the plan of care is consistent with patient choice, and that patient needs are communicated to the relevant healthcare team, Identifies and advocates for needed resources and services for the patient.
  
• Enhance a collaborative relationship with clinical staff to maximize the patient's and family's ability to make informed decisions
  
• Prepares and maintains required documentation and patient referral statistical database in accordance with program policy and procedures.
  
• Escorts patients to appointments at physician offices and admissions at participating Tenet facilities and coordination.

  Education:
  
  • 
    
  • High School Diploma + experience in hospital setting.
    
  • Preferred: Bachelor's degree, child life certification.
